Ashley Grade School Student Registration July 27
Students attending Ashley Grade School District No. 15 will register on Thursday, July 27, 2017 in the cafeteria. The hours of registration are 9 am until 5 pm.
Those students registering for PreK will register in the PreK building.
ALL FEES ARE TO BE PAID AT REGISTRATION
Book Rental & Computer Lab Fee (Grades K-8) – $50 per student
Scholastic News/Story Works/Science Weekly (Grades K-8) – $10 per student
Additional supplements may incur additional fees.
P.E. Shirt (Grades 6-8) – $10
Sports/Extracurricular Activity Fee
(1-2 sports/extracurricular activities ) $10 per student
(3 or more sports/extracurricular activities) $25 per student
All parents/guardians will complete an assurance of residency at the time of registration. Families and students NEW to the district must provide two forms which show proof of residency.
Proof of residency includes, but is not limited to: residency questionnaire and affidavit (notarized), lease agreement, mortgage papers, utility bills (in your name), and property tax bill for residence or mobile homes.
Any returning Ashley students who have changed their location of residence, but still reside in the district, must also provide two documents for proof of residency.
Kindergarten students must be five (5) years of age on or before September 1, 2017. All kindergarten students must have a copy of a certified birth certificate, physical with updated immunization record, lead screening, dental exam and vision exam on registration day. (A hospital birth certificate does NOT fulfill this requirement.)
2nd Grade – Dental examinations are required for students entering 2nd grade.
6th Grade – Physical examinations with updated immunization record and Dental Exams are required for students entering 6th grade.
Out‑of‑state students transferring to Ashley Grade School are required to have a dental exam, physical exam, immunization records, and vision exam.
Registration forms, required exam forms and school supply lists are available on the AGS website (www.ashleyccsd15.org/).
Students will NOT be allowed to enroll without the completed forms on file. Students whose physicians have scheduled required immunizations must provide the immunization schedule on the first day of school.
Students new to Ashley Grade School will need to submit a certified copy of their birth certificate and proof of residency. Birth certificates presented by hospitals are not considered official and will not fulfill this requirement.
The first student attendance day will be August 14, 2017 with dismissal at 3:00 p.m. Students should register prior to the first day of school. Buses will run regular routes on August 14.
